SOS evaluates organizations across defined categories. Each category is scored using observable evidence, documented procedures, and on‑site verification.

How does SOS evaluate safety?

  • Physical Safety Controls — access control, lighting, surveillance coverage, hazard mitigation.

  • Emergency Readiness — emergency plans, drills, communication systems, redundancy.

  • Incident History — OSHA logs, internal reports, severity and frequency of past events.

  • Training & Culture — employee training frequency, leadership involvement, safety accountability.

  • Compliance & Documentation — written policies, inspections, maintenance logs, regulatory alignment.

  • Environmental & Operational Risk — layout, equipment safety, environmental hazards, workflow risks.

Each category is weighted and contributes to the final SOS Safety Score.

Understanding your SOS Safety Score

SOS uses a transparent, consistent rating scale:

  • A (90–100) — Strong, proactive safety posture with documented systems and low incident risk.

  • B (80–89) — Solid safety practices with some areas for improvement.

  • C (70–79) — Basic safety measures in place; notable gaps that increase risk.

  • D (60–69) — Significant weaknesses; elevated risk of incidents.

  • F (<60) — Unsafe conditions; critical failures in safety systems.

Ratings are updated annually or after major operational changes.

Disputes & Appeals

Organizations may request a review of their SOS Safety Rating within 30 days of publication.

To submit a dispute, email disputes@stateofsafe.com with:

  • Company name

  • Date of assessment

  • Specific items being disputed

  • Supporting documentation or evidence

SOS will review the dispute, the original assessment notes, and any new evidence. A written decision will be issued within 30 days. Ratings may be upheld, revised, or withdrawn based on evidence.

How SOS maintains neutrality

SOS operates an independent safety‑rating organization. To protect neutrality:

  • SOS does not own, operate, or have a financial interest in any company it rates.

  • Ratings are based solely on documented criteria and verified evidence.

  • Assessors must disclose conflicts of interest and will be reassigned if necessary.

  • Purchasing consulting or improvement services does not influence ratings.

SOS ratings represent an independent, evidence‑based opinion.

  • Annual updates or re‑ratings after major operational changes.

How the process works

  • Submit your organization’s information through the request form.

  • An SOS assessor is assigned and contacts you to schedule the evaluation.

  • The assessment is conducted on‑site and through documentation review.

  • Your score is calculated using SOS’s consistent, evidence‑based criteria.

  • You receive your rating privately before publication, with the option to dispute findings through a formal review process.

  • Once finalized, your rating is added to the SOS public directory.
